*PRINTED copy of a currently dated contract, lease, water bill, electric bill, etc. with your name and physical address. Phone and internet bills will not be accepted.
SHOT RECORD*
*Oklahoma requires Hep A for all school-age children. Must start school with the first shot of a series of two.
*Tdap is also required for all students entering
7th grade and above.
